USDA AMS Livestock & Poultry Program, Quality Assessment Division (QAD) has transitioned to a new system where egg grading results, pre-op sanitation reports, certificates, approved labeling and more will be recorded and stored digitally instead of paper or pdf forms. QAD can now offer customers operational and quality data recorded in the system through the three reports listed below.

• LP-74 Sanitation Report- emailed daily
• LP-75 Daily CandlingRecord - emailed daily
• LP-516 Retention Log- emailed weekly and monthly with cumulative data

To request reports for your plant, submit the following information to your local QAD supervisor. The request must be approved by QAD personnel to ensure the requester should receive information for that plant.

• Plant Number and Name
• Point of Contact:  First and Last Name
• Business email address
• Report Format Requested: PDF, Excel or CSV

QAD is unable to lump multiple plants in one report, but participants can provide multiple email addresses to receive reports. For further questions, please contact your local QAD supervisor or email LP.QAD.DataAnalytics@usda.gov
